AseraCare Home Health provides comprehensive, specialized orthopedic home healthcare services emphasizing rehabilitation therapy, wellness and patient recovery. An orthopedic clinician is assigned to each patient, and serves as a liaison between the orthopedic surgeon and all other disciplines.

An AseraCare Home Health nurse instructs the patient on their medications, wound care, diet and also assesses the impact of other health-related needs. An AseraCare Home Health physical or occupational therapist starts the patient in a progressive exercise program, and then reviews their ability to walk, transfer and perform their daily activities safely and independently. The AseraCare Home Health nursing team is also assisted by a home health aide who helps with the patient's personal care.

Pulmonary Care

For patients with chronic obstructive pulmonary disease (COPD) or other pulmonary conditions, AseraCare’s home health services may help in a variety of ways.
