AseraCare Home Health helps our patients maintain greater independence and achieve a higher quality of life by providing the care services they need within the comfort of their own homes or in an assisted living facility. Whether the services are for you or for a loved one, you might need guidance and support to manage care at home. AseraCare Home Health is prepared to help you by offering a full range of services including the following:

Diabetes Management

Diabetes management means balancing, diet, exercise and medication. AseraCare Home Health helps individuals with diabetes check their blood glucose levels, modify their diets and come up with an exercise program that is sensible and easy-to-follow.

Cardiac Care

For patients who have undergone bypass, angioplasty or pacemaker surgery, AseraCare Home Health offers professional staff skilled in cardiac care. This service may include increasing patient knowledge of cardiac disease and recovery, ensuring that patients and caregivers comply with the medication and rehabilitation program and maintaining and instructing patients on the use of diagnostic equipment.

Neurological Conditions Care

AseraCare Home Health offers a variety of services to individuals with neurological conditions such as Parkinson's Disease, motor neuron disease, stroke, Huntington's Disease and brain injuries. Depending on the patient, services may include physical, occupational or speech therapy.

Oncology Care

AseraCare Home Health offers cancer patients services they may need throughout the course of their disease. Services may be directly related to the cancer and its treatment or around social issues related to the disease.

Orthopedic Care Services

AseraCare Home Health provides comprehensive, specialized orthopedic care services emphasizing rehabilitation therapy, wellness and patient recovery at home. An orthopedic clinician is assigned to each patient, and serves as a liaison between the orthopedic surgeon and all other disciplines.

An AseraCare Home Health nurse instructs the patient on their medications, wound care, diet and also assesses the impact of other health-related needs. An AseraCare Home Health physical or occupational therapist starts the patient in a progressive exercise program, and then reviews their ability to walk, transfer and perform their daily activities safely and independently. The AseraCare Home Health team is also assisted by a home health aide who helps with the patient's personal care.

COPD Care and Management

For patients with chronic obstructive pulmonary disease (COPD) or other pulmonary conditions, home health services may help in a variety of ways. Skilled nursing can assess the patient’s breathing ability, monitor medications, monitor oxygen saturation and lung sounds, and provide education on the management of oxygen levels and possible side effects of medications. In addition, physical, speech and occupational therapy can help improve the patient’s quality of life by helping them adapt to any physical limitations.

Wound Management

To help patients maintain their independence, AseraCare Home Health offers wound care services to assure proper wound management. Services seek to increase patient and family knowledge of would care, prevent future wounds and increase the healing rate of wounds.
