medication. AseraCare Home Health helps individuals with diabetes

For patients who have undergone bypass, angioplasty or pacemaker
surgery, AseraCare Home Health offers professional staff skilled in

Orthopedic Care Services

AseraCare Home Health provides comprehensive, specialized
orthopedic care services emphasizing rehabilitation therapy,
wellness and patient recovery at home. An orthopedic clinician is
assigned to each patient, and serves as a liaison between the
orthopedic surgeon and all other disciplines.
An AseraCare Home Health nurse instructs the patient on their
medications, wound care, diet and also assesses the impact of other
health-related needs. An AseraCare Home Health physical or
occupational therapist starts the patient in a progressive exercise
program, and then reviews their ability to walk, transfer and
perform their daily activities safely and independently. The
AseraCare Home Health team is also assisted by a home health aide
who helps with the patient's personal care.
COPD Care and Management

For patients with chronic obstructive pulmonary disease (COPD)
or other pulmonary conditions, home health services may help in a
variety of ways. Skilled nursing can assess the patient’s breathing
ability, monitor medications, monitor oxygen saturation and lung
sounds, and provide education on the management of oxygen levels
and possible side effects of medications. In addition, physical,
speech and occupational therapy can help improve the patient’s
quality of life by helping them adapt to any physical
limitations.
