Rehabilitation Therapy Services

AseraCare Home Health provides comprehensive, short-term therapy, skilled nursing and restorative care to patients recovering from general injury or surgery, as well as patients with more complex needs including cardiovascular or pulmonary conditions. Our experienced therapists and home health nursing staff work closely with the patient and primary physician to develop a recovery plan that specifically addresses patient needs.

We utilize a unique system that quantitatively measures rehab outcomes. Through it, we are able to show patients their progress as they work toward recovery and adjust their therapy as needed.

By combining the skilled nursing services with innovative physical therapy, speech therapy and occupational therapy programs, patients benefit from a well-rounded recovery plan that helps them to regain their mobility and independence.

Physical Therapy

AseraCare Home Health clinicians realize that physical therapy pertains more to than just being able to sit, stand and walk on one’s own. While our therapists do help patients regain or improve their abilities to do these things, we also help them identify and maximize their movement and flexibility potential as it relates to prevention, treatment and rehabilitation.

Our physical therapy program and home health aide helps patients develop their potential in regards to any or all of the following:

Occupational Therapy

AseraCare Home Health’s occupational therapy program has one goal in mind: To give people the skills they need to achieve a healthy and balanced life.

For example, if you or a loved one is recovering from a stroke, you may need to relearn home management skills such as making the bed, grocery shopping or meal preparation. If you are recuperating from back surgery, you may need to regain your joint mobility or adapt to bathing and showering on your own.

Our occupational therapy program and home health services teach you how to safely use appropriate techniques and adaptive equipment to perform any number of activities, including the following:

Grooming, hygiene, dressing
Meal preparation, feeding, clean up
Mobility
Home management
Shopping
Financial management
Emergency response/safety procedures
Functional communication, phone use
Medication management

Speech Therapy

In addition to assisting you or your loved one regain the fundamentals of speaking, AseraCare Home Health's speech language therapy program aims to help you cope with motor speech disorders, language disorders, swallowing and more. Our speech language pathologists evaluate each person’s communication skills individually, and they will work closely with you to help convey your needs and care for yourself. Our speech therapy program and home health aide has been specifically designed to help people needing assistance with any or all of the following:

Attention to a task with distraction
Long-term and short-term memory
Ability to complete sequenced tasks

Functional reading and writing

Our orthopedic therapy program offers our program-based rehabilitation to patients in a variety of settings. We also work with those closest to you or your loved one, helping them learn techniques that you may need as you strive to achieve a healthier, more active and more fulfilled life.
