Colleen has been with AseraCare Home Health as a Provider Relations Manager for over a year. As a registered nurse and certified pulmonary nurse, Colleen came to AseraCare with a great knowledge of her area’s market in Pennsylvania. During her time with the company, she has been able to use her experience to successfully grow and develop her agency’s relationships with various health care providers. Part of what she does best is making sure that providers understand how AseraCare Home Health can provide their patients with quality care.

As a people person, Colleen’s role fits her perfectly. She loves meeting and working with new people and developing lasting relationships. She believes in the services that AseraCare Home Health provides. When patients need to transition from one care setting to another, such as from a skilled nursing facility to home, AseraCare is able to help make the transition as smooth as possible. “I always promote our continuum of care with Golden LivingCenters, Aegis Therapies and AseraCare Hospice,” she explains.

Colleen’s positive impact on her agency is apparent. She is proud to say that she has received praise and gratitude from many patients and their families. She helped one patient in particular spend one last holiday season at home before moving into an assisted living facility. Her son couldn’t have been more grateful. “He was overwhelmed with gratitude,” Colleen said. “I received a beautiful thank you note from the family. We tend to think of our home health services as only meeting clinical needs. Sometimes they’re much, much more.”
