Case Manager
AseraCare Home Health
Letty has spent the past two years with AseraCare Home Health
serving as a case manager in Florida. In this role, her
responsibilities are broad. On any given day, she may perform
skilled nursing assessments and handle admissions or carry out
discharge assessments and counsel patients for mental health
concerns. She assists in educating new employees, and occasionally,
Letty also teaches in-services to community physicians.
One of Letty's best qualities is her sparkling personality,
which she is able to use to the benefit of her patients. "I was
caught in a situation where there was a major, unexpected family
conflict," she explains. "I acted as the mediator with positive
outcomes. The family resolved a long-term conflict regarding the
care of my elderly patient." Her skills in problem solving and her
training as a counselor allowed her to resolve the situation with
the whole family feeling comfortable.
Letty is driven by her passion to care for others. "The
challenges of caring for patients with Alzheimer's and their
caregivers are very rewarding to me," she says. "I enjoy seeing my
patients smile when they see me. It confirms that you are needed to
help stabilize them - medically, physically, emotionally and
spiritually."
Providing excellent service to her patients is Letty’s number
one priority. "Patience, active listening and being non-judgmental
are important. You will be surprised what you learn later," she
explains. "Senior citizens are loaded with experience and
intelligence; they are a library full of life experiences."
